QA's role

QA should communicate what was tested, what was not tested, known defects, evidence, limitations, and quality recommendation. QA does not need to own every release activity to surface risk in those areas.

Conditional release

A release may proceed with accepted conditions such as enhanced monitoring, feature flag OFF, workaround, restricted rollout, known low-risk defect, or follow-up action.

Make residual risk explicit

Release readiness is a decision system. It should make the remaining uncertainty visible enough for accountable leaders to make an informed choice.
